Required Forms
Please download and complete the following forms prior to registration:
• Concussion Awareness Form
• LCW Player Code of Conduct Form
• LCW Parent Code of Conduct Form
• Physical History, Exam, and Waiver Forms*
*Note: Both physical forms must be printed and signed by a physician.

Sports physicals are only good for one year. If a player received a sports physical prior to May 30, 2025, it will not cover them for the 2026 season since the baseball season ends in May. Any physical dated May 30th, 2025 or later is valid for the 2026 baseball season.

Sports Physicals may be conducted through any physician service you choose. If you are looking for a lower-cost option, sports physicals are available at MEDcare Urgent Care, Premier Medical, and CVS Minute Clinic.
Completed Forms
Completed forms can be uploaded to the online registration form (below), submitted to a coach or board member in person, or scanned and emailed to lowcountrywildcatbaseball@gmail.com.
Athletic Fee
The athletic fee for the spring 2026 season will be $550 for middle school and $650 for varsity. The fees will help cover cost of the field, umpires, uniforms, and baseballs for the season. Registration is not complete until the athletic fee is paid. A payment link can be found at the end of the online registration form.
